Updated: Canon EOS DSLR plus PIXMA PRO9000 or PRO94500 Mark II Printer Combo Rebate

Rebate forms are now showing up for the Canon EOS DSLR and PIXMA PRO9000 and PRO94500 Mark II Printer Combo Rebates. Note that the referenced form has two pages - the PRO9000 rebate is on the second page.
 
The rebate amounts are significant - $400 and $500 respectively. Qualifying cameras include the EOS 7D, 60D, T3i, T2i and T3.
 
B&H once again has the PIXMA PRO9000 Mark II in stock again, but the price is back to normal. I do not see the rebate form on B&H's site yet. Adorama and Amazon.com both have the rebate forms available, but only Amazon.com has stock and the form.
 
Combine the new Canon EOS DSLR rebates with printer rebate for a great deal.
